Fit-and-Proper & Disclosure Declarations *

1. Have you ever been convicted of a criminal offence involving fraud, dishonesty or moral turpitude, in Ghana or elsewhere?

2. Are you currently the subject of any criminal investigation or prosecution?

3. Have you ever been disqualified, removed or restricted from acting as a director, trustee or public officer?

4. Have you ever been sanctioned, censured or fined by a professional body or regulator (e.g. Bank of Ghana, SEC, ICAG, GBA)?

5. Have you ever been declared bankrupt or insolvent, or entered into an arrangement with creditors?

6. Has any company of which you were a director entered receivership, administration or liquidation during, or within two years after, your tenure?

7. Are you aware of any conflict of interest that could affect your participation in the programme or in cohort case work?

8. Is there any other matter that could reasonably affect your fitness to be certified as a director by the Institute?
